Mail-order aligners: what is missing from the box

Providers who ship aligners based on a do-it-yourself impression or a phone photo advertise price and convenience. What falls away is the diagnosis: no X-ray, no assessment of the roots, no examination of the periodontium, no check of the jaw joint.

This goes unnoticed as long as nothing happens. It becomes noticeable when a tooth with untreated periodontitis is moved, when a root reaches the limit of the bone, or when a tooth is moved that would first need root canal treatment.

Supervision is also missing during the treatment: whether a tooth is actually moving as planned can only be seen if someone looks. If the movement deviates, the plan has to be changed – a set of aligners shipped in full in advance cannot do that.

Anyone comparing prices should therefore compare what is included. The cheapest treatment is not the one that costs the least, but the one that does not have to be repeated.
